~ Purpose ~

Marine accidents and incidents require a systematic and objective investigation to determine their underlying causes and identify measures to prevent recurrence. In accordance with the applicable International Maritime Organization (IMO) guidelines and the Code of the International Standards and Recommended Practices for a Safety Investigation into a Marine Casualty or Marine Incident, investigations follow an established process for collecting and analyzing evidence, identifying contributing factors, and developing effective safety recommendations. A thorough investigation helps improve safety awareness, strengthen operational procedures, and reduce the likelihood of similar incidents in the future.


~ Objectives ~

Participants will gain an understanding of the systematic approach to investigating marine accidents and incidents. The course covers investigation planning, evidence collection and preservation, gathering relevant records and witness statements, interviewing involved personnel, and analyzing information to identify root causes and contributing factors. Participants will also learn how to document findings, prepare investigation reports and case studies, and develop corrective and preventive actions with practical action plans. Particular emphasis is placed on objective analysis, accurate documentation, and using investigation findings to improve maritime safety and prevent recurrence.
